Adam is trying to find out what Amina has been suffering from.  He brings her to the hospital in Austin, but as they're being told that she can't be admitted because it's not an emergency and she has no insurance.  She pulls out an insurance card from Egypt, but the company's assets have been frozen out of suspicion of corruption.  Both Adam and the hospital admin stop arguing when they see the look of fear on her face.  Adam asks her what's wrong and suddenly realizes that it's because there are several men with long black beards, thumps on their heads and skull caps.  Adam deduces that they're looking for her.  Is Amina pregnant?  Do they scape from the hospital?  Where do they go?  West Texas?  Andrews?  The toxic waste facility.  Is she ultimately a figment of his imagination?  Has Adam been working this entire time there?
